Social Worker 2 (SW 2)- Expert

Pay Rate: $28 / hour

1.The BCC MAT Social Workershall communicate with SCI MAT Social Worker when reentrants OD in communityand are returned to SCI for detox to determine eligibility and appropriateness forMAT.

2.The SCI MAT Social Worker shall assist with MATreferrals in the community, including setting up and scheduling appointmentsfor medication and treatment.

3.The BCC and SCI MAT Social Worker shall ensureneeds, benefits, and services are obtained in accordance with assignedindividuals' treatment plans.

4.The BCC and SCI MAT Social Worker shall trackand update spreadsheet/database on everyone released from an SCI on a MAT.

5.The BCC and SCI MAT Social Worker shall attendweekly Vivitrol Conference Call, and other meetings as needed.

6.MAT Social Workers shall document all contactsin appropriate systems (Captor, Sapphire etc.).

7.MAT Social Workers shall attend all requiredtrainings.

8.MAT Social Workers shall meet with reentrants withpositive urine drug screens to assess and refer to treatment.

9.MAT Social Workers shall complete the GPRA asrequired (Initial, 6 months, discharge).

10.MAT Social Workers shall meet with Reentrants inthe SCI, BCC, Parole Offices, and/or Treatment providers as necessary.

11.MAT Social Workers shall communicate all treatmentplanning to Center Staff, SCI Staff, and/or Parole staff.

12.MAT Social Workers shall subscribe to the MAT/AODTreatment framework and policies and procedures of the DOC.

13.MAT Social Workers shall implement and providesocial work, counseling, and case management services to individuals to enhancetheir social functioning and to help them attain a more satisfactory social,economic, or physical adjustment.

14.MAT Social Workers shall perform in accordancewith applicable rules, regulations, policies, and procedures under the generalsupervision of an administrative supervisor.

15.MAT Social Workers shall assess offenders'psychosocial strengths, weaknesses, and social work service's needs, and eligibilityfor services.

16.MAT Social Workers shall counsel individualsutilizing various treatment modalities.

17.MAT Social Workers shall discuss assignedindividuals' psychosocial assessments and family history in multidisciplinarytreatment team meetings.

18.MAT Social Workers shall present with thefollowing skills:

a.Knowledge of professional social workprinciples, practices, and methods.

b.Knowledge of professional casework principles,practices, and methods.

c.Knowledge of individual and group behavior.

d.Knowledge of counseling techniques andinterventions used to recognize or improve psychosocial, economic, andphysiological situations of individuals and families.

e.Knowledge of commonly occurring physical andmental illnesses and their social, economic, and psychological implications.

f.Knowledge of social, economic, health,habilitative, and rehabilitative problems, treatment methods, and resources.

g.Knowledge of the requirements of the HealthInsurance Portability and Accountability Act (HIPPa) as well as state confidentialitylaws as applied to Social Work practice.

h.Ability to read and interpret rules,regulations, policies, and procedures relating to SoCal work services andprograms.

i.Ability to establish and maintain effectiveworking relationships.

Ability to communicate effectively in writing

k.

Ability to communicate effectively orally.
