Health Advocates Network Physical Therapist - Home Care Danville in Champaign, Illinois

General Summary:

The Physical Therapist at OSF Home Care is responsible for visiting patients in their home to perform therapeutic evaluations, exercises and treatments to restore function and prevent disability following illness or injury as ordered by a physician and in accordance with State Practice Act and Home Care rules and regulations.

Demonstrates the knowledge and skills necessary to provide patient care that is appropriate to the ages of the patients served. This requires that the clinician demonstrates knowledge of the principles of growth and development, as well as physical, emotional, and psychosocial needs of the patient population served.

Duties and Responsibilities:

Evaluates patients' physical and functional status including musculoskeletal system, neurological system and functional mobility skills in activities of daily living and includes gathering information from the medical record, patient, and family/care giver.

Analyzes evaluation results and sets appropriate short and long term goals to support/develop the patients' plan of care.

Selects, administers, or monitors treatment. Treatments may include: modalities, therapeutic exercise, transfers, gait training, functional mobility training, and home safety.

Determines appropriate assistive devices such as transfer aides, ambulation aides, orthotics, and prosthetics which are needed for functional mobility skills; including patient and family teaching as needed.

Reassesses patients' response to treatment and adjusts program accordingly; includes ongoing communication with the physician, physical therapist assistants, patient and/or family and other health care disciplines.

Recommends other disciplines, programs, and equipment/ supplies as needed by the patient and/or family.

Evaluates effects of physical therapy treatments and updates plan of care as needed.

Maintains accurate and timely documentation in electronic medical records.

Determines, instructs, and issues appropriate home programs.

Adheres to universal precautions and infection control policies.

Attends to the physical, emotional, as well as psychosocial needs of the patient population served.

Attends patient care conferences, in-service education classes, and meetings as required. Keeps informed of developments in the field of Physical Therapy and related disciplines in order to maintain and enhance professional expertise.

Develops and maintain a positive working relationship with department and other Home Care personnel, patients, students, and outside agencies.

Supports and is involved in Home Care's continuous quality improvement efforts designed to improve patient outcomes and patient satisfaction in an efficient manner.

Knowledge, Skills and Abilities Required:

? Requires current state license to practice as a Physical Therapist.

? One year recent physical therapy experience preferred.

? Home healthcare experience preferred.

? Must have CPR certification prior to patient contact.

? Must be able to use and operate therapeutic equipment

? Must have knowledge of the treatment of physical disorders, have an understanding of problem solving and decision making skills.

? Proficient with electronic documentation, Microsoft programs including, Outlook, Word, etc.

? Provides own transportation with auto insurance and abides by all laws, rules and recommendations to comply with OSF Safe Driving Policies while on duty. Must maintain a valid driver's license.

? Work schedule may include some weekends and holidays as needed.

? Must be able to work independently in virtual work environment and execute own workload.

? Excellent communication and interpersonal skills required.
